Client Service Referral Policy

At Community Wellness Outpatient Mental Health Center, we are committed to providing comprehensive, person-centered care that supports the holistic needs of each individual we serve.

Please note: Community Wellness Outpatient Mental Health Center does not directly provide housing of any kind. However, we refer and coordinate housing-related needs when expressed by participating clients to both affiliated and non-affiliated service providers throughout the Baltimore area, including the organizations listed in this document.

We recognize that no single program can address all the needs a client may experience during their recovery journey. Therefore, if a participating client expresses a need for services beyond our scope—such as specialized medical care, housing assistance, or social support—we will make every reasonable effort to refer the individual to a qualified external provider or community-based resource.

Our goal is to promote continuity of care and support each client’s progress toward wellness, stability, and independence. Freedom Healthcare collaborates with a trusted network of providers and maintains an updated list of community resources.

Homelessness and Recovery

We recognize that housing instability is a critical barrier to health and recovery. Research shows that stable housing improves mental health outcomes, increases treatment adherence, and reduces hospitalizations.

Clients and referring professionals may contact our administrative office for assistance with linkage to housing or other community-based services.
